All of our forms are available upon registration with the client web portal and will be sent to you once you book an appointment. All forms must be filled out prior to arriving for your appointment. Here is a direct link to those forms if the web portal is not working for you.

*Note - We are not responsible for keeping track of eligibility, balance or payment if the direct billing claim is denied. *If links do not work, or if you do not have access to a printer, please arrive 15 minutes early to fill out the proper paperwork.

Location
Located at 4104 50th Avenue. Old wilson registry building - Brick building, Henka Law & Sunlife Financial are in the same building. Park & enter on the East side (Highway Entrance)
- Please find a comfortable seat in the lobby and Janelle will come and get you at your appointment time.
- If you had any issues filling out the required paperwork prior to arriving for your appointment, please fill in the forms located in the lobby.
Cancellation Policy
Cancellations happen, we get it. Our goal at Rivia Therapeutics is to ensure that we can provide the best care for our patients and because of this we do strictly enforce our cancellation no-show policy. A more detailed policy will be sent out with your intake form, but here are the basics.
- 24 hours minimum notice. Less that 24 hours notice makes it very short notice for us to fill the appointment slot. Cancellations over the weekend for Monday appointments are not accepted.
- Cancellations must be made from your online account (rivia.noterro.com) or by phone/text 780-871-1202
- If you are more than 15 minutes late for an appointment without attempting to reach us via phone or text may result in a "missed appointment". We are not obligated to make up for time lost due to tardiness and there will be no discounts or reduced fees for being late (i.e a 60 minute appointment will not be charged out as a 30 minute appointment if you only show up for the last 30 mintues). Depending on the type of appointment, you may be rescheduled for later in the day or later in the upcoming weeks.
- Missed appointments incur fees that must be paid prior to your next appointment. These fees can not be paid by your 3rd party insurance. Fees can range from half to full price of service requested. Any missed appointments for out of town services will automatically be charged full price of service requested and require non-refundable deposits in the future.
- Many missed appointments may result in:
- loss of direct billing services (a receipt will be issued to you to submit to your insurance after your appointment/services rendered)
- loss of online booking capabilities (must book by phone only)
- require non-refundable deposit at time of booking (a receipt will be issued to you to submit to your insurance after your appointment/services rendered)
- require non-refundable payment in full (a receipt will be issued to you to submit to your insurance after your appointment/services rendered)
- termination of services
- Emergency cancellations:
- Things happen. Let us know asap that you cannot make your appointment. It's up to the discretion of the practitioner to issue a charge depending on the situation. (Generally, no charge; however frequent "emergency cancellations" will eventually be treated as missed appointments).
Follow ups
It's HIGHLY recommended that a follow up appointment is booked 1-2 weeks after the assessment.

- Ensure that everything is going well and homecare is still effective
How often will you need to follow up? This answer really depends on you! If you are keeping up with your homecare & appointments you should be feeling much better and we should be attaining your goals much quicker.
Generally for minor issues the assessment and a follow up can be enough. Sometimes a few more sessions are beneficial.
